The ‘Ai Pono Outpatient Program is the longest running intensive outpatient eating disorder treatment program not only in Hawaii, but in the United States.
Our outpatient program has a long history. Its origins were as the Anorexia and Bulimia Center of Hawaii (ABC Hawaii), co-founded by Drs. Anita Johnston and Steve Orenstein in 1982. Initially a “center without walls” it began as a collection of professionals in Oahu specializing in the treatment of eating disorders.
Recently, we have received numerous requests from women residing in Hawaii who are need of an outpatient level of care, but who are unable to make it to our Maui or Honolulu locations.
So now, we return to our roots – expanding beyond the traditional walls of a treatment center by offering online outpatient programming for Hawaii residents.

Our online IOP program is perfect for…
Women who have completed treatment at ‘Ai Pono Hawaii at the residential or PHP level of care who are currently unable to participate in in-person outpatient treatment
Those who have completed treatment elsewhere at the residential or PHP level of care and are in search of a local provider for outpatient services in Hawaii
Those who live on neighboring islands outside of Oahu or Maui who are unable to travel to our physical site to attend programs
Those with personal or familial obligations which prohibit them from attending in-person programs
Those who are unable to leave home to attend programs
Individuals who may be reluctant to receive treatment in-person

Our online IOP Program format is nearly identical to our in-person program and consists of 3x three-hour sessions per week including: therapy groups; nutrition groups; and meal support.
Patients enrolled in our Online IOP receive the same evidence-based treatment that we deliver via our on-site eating disorder treatment programs and all facilitators have extensive experience not only in the eating disorder treatment field in general, but also with Dr. Anita Johnston’s unique philosophy and approach.

What is a virtual intensive outpatient program (IOP) for eating disorders?
A Virtual IOP is a structured, part-time level of care delivered online. Clients participate in multiple therapy sessions per week—while continuing to live at home—receiving support for eating disorder recovery without needing residential treatment.
Who is a good fit for virtual IOP?
Virtual IOP is ideal for individuals who are medically stable but still need more support than traditional outpatient care. It can also be a step-down from residential or PHP, or a step-up from weekly therapy.
What does a typical virtual IOP schedule look like?
All clients start by attending the program four days per week (Monday-Thursday) from 5:00pm-8:00pm. ‘Ai Pono’s Virtual IOP is a group-based program primarily, but clients do receive one monthly individual session with a dietitian. Groups range in topics including process groups, nutritional education groups, practical recovery skills, and more.
How are meals supported in a virtual setting?
The evening program includes a break for dinner. Clients are allowed to turn off their cameras while eating and have the opportunity to process their experience during and following their meal.
What types of therapy are offered in virtual IOP?
Our Virtual IOP is rooted in many of the same concepts and approaches as the residential program including the foundational work of Dr. Anita Johnston, CBT, DBT Skills, intuitive eating, and others.
Can co-occurring mental health conditions be treated in virtual IOP?
Yes, Virtual IOP addresses co-occurring conditions like anxiety, depression, and trauma alongside the eating disorder, however direct management of these by an individual therapist or psychiatric provider are not a feature of Virtual IOP. For clients needing more support in these areas, the residential program may be a better fit.
How does virtual IOP maintain accountability and structure?
Even though treatment is remote, programs maintain structure through consistent scheduling, attendance expectations, meal support, and regular check-ins with the care team. Our Program Supervisor also often coordinates with a client’s outpatient providers (PCP, therapist, etc.) to provide comprehensive support to the client.
What are the benefits of choosing a virtual IOP?
Virtual care allows clients to receive intensive support while staying connected to their home, work, or school life. It also provides opportunities to practice recovery skills in real-world settings.
What technology or setup do I need to participate in the Virtual IOP?
Clients need a reliable internet connection, a private space, and a device with video capability. We use a secure, HIPAA-compliant platform (Zoom) to protect confidentiality.
How long does virtual IOP last, and what happens after?
Length of stay varies based on progress, but most clients participate for a few months. We offer a unique option at ‘Ai Pono as well to slowly titrate down in support (i.e. moving from four days a week, to three, two, or one). Afterward, clients typically transition to outpatient care with a personalized aftercare plan.
